How to Refer Your Child to a Speech Pathologist at Small Sprouts Therapy

Are you thinking about referring your child to a speech pathologist? At Small Sprouts, we make the process straightforward and stress-free! Here’s everything you need to know about getting started.

Do I Need a GP Referral?

Nope! You can self-refer to all Speech Pathology services. At Small Sprouts, we specialize in providing evidence-based assessment and therapy for children aged 0-12 years.

How to Refer Your Child

Referring a child to our clinic is easy. You can:

  • Give us a call

  • Send us an email

  • Complete our expression of interest form online on our website

What to Expect During the Enquiry Process

When you contact us, our friendly receptionist will ask for the following information:

  • Child's full name

  • Child's date of birth

  • Representative's contact details

  • Whether the child is an NDIS participant or a private client

  • If NDIS participant: whether they are self-managed or plan-managed

  • If a private client: whether they have a GP Management Plan

  • Previous engagement with a speech pathologist

  • Concerns regarding the child's speech and language development

Next Steps After Your Enquiry

After the initial enquiry, you’ll receive an electronic case history (onboarding) form and a fee schedule. Completing and returning the case history form promptly is essential to confirm the referral. This form gives our therapists important background information and includes consent questions and our cancellation policy.

You’ll also need to send any reports or letters from your child's doctor, paediatrician, or other allied health professionals. This helps us take a holistic approach to your child’s care. Please return all case history forms within 14 days to ensure your child is successfully placed on our waitlist. If we do not receive the completed case history form within this timeframe we are unable to process the referral and your child has not been placed on the waitlist. 

Behind the Scenes: What We Do

Our receptionist will input your child's details into our online system and add them to our waitlist. When an appointment becomes available, we’ll contact you to schedule an initial assessment. You’ll receive a confirmation email for this appointment. 

For NDIS Participants

Before the initial consultation, NDIS participants will need to email us a copy of their NDIS goals and participant number. This information is crucial for our therapists to review and for invoicing purposes. Please note Service Agreements will be created and sent after the initial consultation.

For Private Clients

Private clients will be asked to review and return a signed Private Client Onboarding form before the initial consultation. 

Initial Assessment and Ongoing Therapy

After the initial assessment appointment, the therapist will discuss ongoing therapy recommendations and outline what to expect next.
